Case A: heat above, cold below
Observational cluster: evening facial heat, cold feet, dry mouth, difficulty settling to sleep and gradual change over several months.
One-label reading: a yin-deficiency pattern may organise several findings efficiently.
Layered questions: L4 asks about fluids and nourishment; L8 asks whether activating fire is displaced; L9 asks about environmental aggravation; L10 asks why the pattern changed when it did. The layers do not cancel the first interpretation. They expose what it does not yet explain.
Record update: any later observation must be used to revise the model, not merely to defend the original label.
Case B: chronic sleep interruption with digestive heaviness
Observational cluster: repeated waking, daytime heaviness, post-meal drowsiness, loose stools, irritability and a complex tongue-pulse picture.
Competing readings: a liver-centred excess account and a spleen-centred deficiency account may each fit part of the presentation.
Layered questions: L3 preserves simultaneous zang-fu relationships; L4 records accumulation and deficiency without forcing either to disappear; L7 tests regional distribution; L10 records chronicity and sequence.
Research value: instead of asking which single label wins, the record asks what evidence would distinguish the alternatives.
Case C: fatigue that changes with season
Observational cluster: fatigue and heaviness that worsen in humid weather, with digestive vulnerability and recurrent seasonal timing.
One-label reading: qi deficiency can describe an important part of the presentation.
Layered questions: L2 records precipitating and maintaining factors; L4 distinguishes deficiency from fluid accumulation; L9 makes humidity and place explicit; L10 tests whether the seasonal sequence repeats.
Why it matters: the environment is not an optional footnote when it changes the meaning and timing of the other findings.
How to read an MLMN case
- Separate observations from interpretations.Do not rewrite a reported or observed finding as a diagnosis.
- Map what is present—and what is missing.A blank layer is not automatically irrelevant.
- Compare more than one account.Record supporting findings, contradictions and the evidence still required.
- Let time test the account.Observed change may support, weaken or transform the earlier hypothesis.
